9 EW NEWS & OPINION May 2018 World view Navigating the red eye M ost ophthalmologists while on call will encounter patients with a red eye. Even in the office, referrals to see patients with chronic red eyes can be a common occurrence. It is important to be able to properly diagnose the etiology for the patient's red eyes and recommend appropriate therapy. Otherwise, these patients can go with a missed diagnosis for a long time and become frustrated, which is a poor practice builder. At times, hygiene and contact precautions must be taken as well to avoid epidemic spread of severe viral conjunctivitis, so an accurate diagnosis is crucial. In this issue of EyeWorld, we review the basics of managing the red eye and hope that it is a welcome topic. We begin with Francis Mah, MD, Christopher Starr, MD, and David Verdier, MD, who will discuss how to diagnose, treat, and prevent the spread of viral and bacterial conjunctivitis. They delve into how to differ- entiate between the two entities and other masqueraders not to miss. Next, we have Leonard Bielory, MD, Vincent de Luise, MD, Bennie Jeng, MD, John Sheppard, MD, and Dr. Starr review a guide to treating seasonal allergic conjunctivitis. They provide some great pearls about how to determine the type of allergy the patient has, how to encourage avoid- ance of allergens, what medications are best to start with, appropriate use of steroids, when to treat associated dry eye disease, which other medical specialists should help with comanagement, how to advise patients to persist, and ideal follow-up regimens for these patients. Hyperacute bacterial conjunctivitis can be sight- and life-threatening and must not be missed. Drs. de Luise and Mah review the bacterial or- ganisms one should consider, how to diagnose appropriately, and the best treatment options for these patients. Chronic conjunctivitis can be a frustrating entity for patients and the clinician if one does not have a methodological approach. Dr. de Luise, Anat Galor, MD, and Mark Mannis, MD, discuss the differential diagno- ses to keep in mind, how a papillary versus follicular clinical appearance can help direct the ophthalmologist, when a culture may be warranted, what not to miss, available therapies, and proper patient follow-up. Finally, there are structural causes of a red eye to remember. The lids, lashes, and conjunctiva are important parts to the ocular surface, too. Kenneth Beckman, MD, and Harmeet Gill, MD, comment on the diag- nosis and management of various common and uncommon entities such as lid retraction, ectropion, entropion, lid laxity, lagophthalmos, aberrant lashes, punctal stenosis, deep fornix syndrome, floppy eyelid, superior limbic keratoconjunctivitis, and conjunctival chalasis.
