The Run for Sight volunteer team from Johnson & Johnson Vision Source (all): ASCRS 13 UPDATE In the journal Combined glaucoma and cataract surgery: Comparison of viscocanalostomy, endocyclophotocoagulation, and ab interno trabeculectomy Sasan Moghimi, MD, Nikoo Hamzeh, MD, Massood Mohammadi, MD, Nassim Khatibi, BS, Christopher Bowd, PhD, Robert Weinreb, MD In this retrospective case series, investigators considered how eyes with open an- gle glaucoma in conjunction with cataracts fared while undergoing phacoemul- sification/viscocanalostomy, phacoemulsification/endocyclophotocoagulation (ECP), and phacoemulsification/ab interno trabeculectomy. Eyes underwent one of the three procedures, with 46 eyes undergoing phacoemulsification/ viscocanalostomy, 35 undergoing phacoemulsification/ECP, and 28 undergoing phacoemulsification/ab interno trabeculectomy. Investigators determined that the group with the lowest IOP at last follow-up were those who underwent pha- co/viscocanalostomy, with a 29% pressure decrease. The phaco/ECP group, with its 20% decrease, was comparable to the 15% decrease found with the phaco/ab interno trabeculectomy technique. Phaco/viscocanalostomy showed the greatest reduction in the number of medications prescribed with a 77% decline, followed by phaco/ab interno trabeculectomy with a 44% decrease, and phaco/ECP with a 40% drop. The fewest complications were seen in the phacoemulsification/ ab interno trabeculectomy group. Investigators concluded that while IOP was lowered with each of the procedures, the largest drop in pressure as well as the greatest reduction of medications was seen in the group that underwent phaco/ viscocanalostomy. The eyes that had the fewest complications were those that underwent ab interno trabeculectomy. Now, newer models are incorporating the accommodative ability of the eye. Such mod- els could ultimately be used for varying research purposes, as well as for testing multifocal intraocular lenses. Other possible uses for such newer schematic mod- els include considering how optical aberrations may affect the image quality and examining the impact of refractive surgical procedures on optical performance. Long-term outcomes for pediatric patients having transscleral fixation of the capsular bag with intraocular lens for ectopia lentis Julia Byrd, MD, Marielle Young, MD, Wei Liu, MPH, Yue Zhang, PhD, David Tate, BS, Alan Crandall, MD, Leah Owen, MD The aim of this retrospective case series was to examine when to use preoperative spectral domain optical coherence tomography (SD-OCT) in patients undergo- ing cataract surgery and consider the cost effectiveness of this. Of the 401 eyes included here, 70 were considered to be normal on the fundus exam, while 107 were found to have an abnormal macular on the SD-OCT. There was a higher rate of such abnormal maculars found in those over age 70 with near vision less than Parinaud 2. The never fiber layer SD-OCT was found to be abnormal in 14.2% of eyes. Of these, there was no history of glaucoma in 30% of eyes and no detected anomalies on fundus exam. When it came to finding anomalies, investigators de- termined that preoperative SD-OCT scanning prior to cataract surgery was signifi- cantly more effective than fundus examination. They also found that higher rates of abnormal macular SD-OCT were associated with older age and near vision that was worse than Parinaud 2. As for an abnormal retinal nerve fiber layer, this was associated with older age and a history of glaucoma. EW May 2018
