Clinical implications Dr. Sharma pointed out that for some there is a big disconnect between having dry eye and the signs and symptoms. The tear osmolarity test can be an objective way of showing patients that they do indeed have dry eye, which can get worse over time. For migraine sufferers, the hope is that by alleviat- ing the dry eye, this may help the headaches. "If you can reduce one of their trigger factors, we might be able to avert a migraine attack or reduce the symptoms of migraine," he said. Dr. Dodd concurred. "When you look at migraine patients, they have other triggers. We don't know that dry eye disease or eye patholo- gy directly trigger migraine, but it's possible," she said. In her view, knowing that patients have migraines may be a tipoff for ophthalmologists to offer a tear osmolarity test. "We think that those with migraines are more likely to have dry eye, and other studies have supported that," she said. "When you do the osmolarity test, you're more likely to have a true positive rather than a false positive." From a testing perspective, this can be useful, she thinks. Dr. Sharma said that when dealing with migraine sufferers, dry eye is something that practitioners should keep in mind. He thinks it makes sense to offer these patients the OSDI questionnaire and possibly a tear osmolarity test. If this indi- cates dry eye, treatment should be initiated, Dr. Sharma thinks. Dr. Dodd said further study on the relationship between dry eye and headaches would be useful for both the neurology and the ophthal- mology communities, as well as for patients.
