World view Rebirth of laser vision correction A nyone who attended the 2017 ASCRS•ASOA Symposium & Congress in Los Angeles knows that refractive surgery is alive and well, and the future looks even brighter. At this meeting, ASCRS put the "R" back into its name with dozens of courses and seminars, highlighted by the inaugural Refractive Day that was attend- ed by almost 1,000 ophthalmologists. Douglas Koch, MD, delivered the first annual Steinert Refractive Lecture to a standing ovation when he was presented with the Steinert Medal by none other than Roger Steinert, MD, himself. In this issue of EyeWorld, we continue the momentum with some ex- traordinary articles highlighting advances in LASIK, PRK, and the new kid on the block SMILE. Make no mistake—these cutting-edge, state-of-the-art procedures are safe, predictable, and efficacious. There are three new refractive corneal procedures that have become available in the U.S. over the past year. Edward Manche, MD, Stephen Slade, MD, Vance Thompson, MD, and William Wiley, MD, discuss advanced wavefront aberrometry LASIK with the iDesign, topographic laser treatments with Contoura, and the SMILE procedure that offers small incision refractive surgery. The one thing that they all agree on is that the availability of new technology will bring better outcomes and more patients into our offices. Refractive surgery has always been a therapeutic treatment to improve patients' quality of vision. Topographic ablations can improve both uncor- rected and best corrected visual acuity. Patients with keratoconus can now be treated with crosslinking and topographic laser ablations and end up wearing their contact lenses more successfully and often even change to glasses and occasionally spectacle freedom. Two of the international pioneers in topo- graphic ablations, John Kanellopoulos, MD, and Raymond Stein, MD, provide pearls for treating these patients while I share the early experience here in the U.S. As previously mentioned, advances in LASIK have been extraordinary. There have been three major studies published in the last year that are dis- cussed by the authors or participants in these papers. Kerry Solomon, MD, is the lead author of a literature review that evaluated every LASIK publication over the last 10 years. 1 Elizabeth Hofmeister, MD, and Daniel Durrie, MD, discuss the PROWL study, which is the single most comprehensive LASIK trial. 2 Finally, Francis Price, MD, was the primary author on the largest prospective trial of patient satisfaction, quality of vision, and complications comparing contact lens wear to LASIK. 3 Now that we know how good refractive corneal surgery is, I would ex- pect that more surgeons are thinking about adding refractive surgery to their surgical armamentarium. Three leading refractive surgeons, Kathryn Hatch, MD, Gregory Parkhurst, MD, and Elizabeth Yeu, MD, share how and why surgeons should add refractive surgery to their practices. They discuss how to get started and the best ways to be trained, and provide tips for performing PRK and LASIK after cataract surgery.
