Reporting from the Asia-Pacific Academy of Ophthalmology (APAO) Congress March 1–5, 2017 Singapore EW MEETING REPORTER 176 therapy, evaluate the ocular surface in relation to vasculature, and moni- tor infections or inflammation. Current limitations include issues concerning OCT image acqui- sition, projection artifacts, eye mo- tion, image processing and display, and segmentation. Segmentation is a particular problem. Current systems assume they are acquiring retinal images; as such, surgeons need to get the patient to look in the right direc- tion to ensure the curved corneal surface being captured appears "flat" and ensure that the segmentation is accurate before accepting image capture. The use of OCT angiography, Dr. Ang said, is in its early develop- mental stages but promises to be a truly disruptive technology, increas- ing understanding of the physiol- ogy and pathology of the vascular anatomy of the cornea, as well as a potential adjunct to therapy. Experts share their pearls for refractive cataract surgery An informative, fast-paced sympo- sium had speakers giving their pithy top pearls for refractive cataract/IOL surgery. Toric IOL challenges Daniel Black, MD, Queensland, Australia, gave his four pearls for managing toric IOL challenges. • Conduct keratometry on virgin corneal tear film. It's important to remember that keratometry is a reflection of the tear film, Dr. Black said. As such, that tear film needs to be in its best possible state. If the patient comes in and undergoes a variety of other tests and exams prior to keratometry, the tear film is in such a state that it won't produce accurate ker- atometry measures, Dr. Black said. It's also important to optimize the tear film by treating dry eye con- ditions and encouraging patients to avoid contact lenses prior to taking these measurements. • Use a capsulorhexis marker. Consistency with your size and centration of the capsulorhexis is important, Dr. Black said, espe- cially with the IOLs on the market April 2017
