"Personally, I tend to perform MIGS procedures in patients who have mild disease that is relatively stable, on one or two drops. I reserve MIGS for patients who I feel con- fident will be okay with a pressure of about 15 to 20 mm Hg. We are experiencing an exciting time in glaucoma right now, and I think that MIGS trials have provided good data. We have some great random- ized clinical trials with the Hydrus (Ivantis, Irvine, California), the iStent, and most recently the CyPass (Alcon, Fort Worth, Texas), which just received FDA approval. But I think we are still waiting on long- term data to better define the safety and efficacy of these devices," she told EyeWorld in an interview on her outlook on MIGS. Dr. Vinod states that the XEN (Allergan, Dublin, Ireland) gel stent is the closest MIGS procedure to trabeculectomy in that it also diverts aqueous to the subconjunctival space creating a filtration bleb. Because the normal outflow system is bypassed, greater IOP reduction can be achieved, which can be crucial in advanced glaucoma cases. She explained, "Some of the newer subconjunctival procedures like the XEN gel stent, which was just FDA approved, tends to get lower pres- sures than the trabecular meshwork and canal-based procedures. It is sort of a hybrid between trabeculectomy and tube shunt surgery, so it holds a lot of promise." Dr. Vinod favors trabeculectomy for her patients with moderate- to advanced-stage glaucoma, includ- ing her normal tension glaucoma patients who may require IOP in the single digits. Also, in patients who are actively progressing or who she feels are likely to progress, trabeculectomy is more effective at reducing IOP than MIGS. She ex- plained, "At this point in time, the MIGS procedures that are currently available won't cut it for moderate- to advanced-stage glaucoma patients who require procedures that can get them to a pressure that is below the episcleral venous pressure. I think that future generations of these de- vices will hopefully provide us even better pressure lowering and allow us to tailor care to the individual pa- tient's ocular anatomy and personal needs."
