World view Improving the safety and efficacy of military personnel and first responders A lmost 1 in 5 refractive surgery proce- dures performed in the U.S. is in the military. The surgeons who perform these procedures are among the most qualified ophthalmologists in the world and have 1 overriding goal: to improve the safety and efficacy of our armed forces. Of Air Force personnel alone, 25% percent have under- gone refractive surgery. Vision no longer limits access to certain occupations, so there is a better applicant pool. In this issue, several pioneers in military re- fractive surgery, including Steven Schallhorn, MD, Charles "Chaz" Reilly, MD, and Frank Butler, MD, discuss the early days of LASIK and PRK in the military and how it has changed our military. These ophthalmologists are the heroes behind the heroes. For many years PRK was the preferred form of refractive surgery in the military because of the fear of flap dislocation, but now femtosecond laser flaps are addressing this concern. Colonel Matthew Caldwell, MD, and Lt. Cmdr. Hunter Stolldorf, MD, discuss the role of LASIK and PRK in the different branches of the military as well as the role of phakic IOLs. Captain Elizabeth Hofmeister, MD, has been a leading proponent of refractive surgery. As the lead researcher of the PROWL-1 trial, which is the most definitive study of LASIK ever performed, she has added greatly to the literature on the safety and efficacy of LASIK. Another important study was performed by David Tanzer, MD, on the quality of vision in Naval aviators. Both of these pioneering ophthalmologists discuss the role of LASIK and PRK in the military and provide a behind-the-scenes look at the data from their landmark trials. Finally, there are many first responders who put their lives in harm's way daily. These firefighters, police officers, and others make our communities safer, and in many cases they do their job better because of LASIK and PRK. Two leading refractive surgeons, Daniel Durrie, MD, and John Vukich, MD, discuss the importance of refractive surgery in this group. In addition, this issue of EyeWorld has an important breaking news item that is a must-read. There is overwhelming evidence that intracamer- al antibiotics reduce the risk of endophthalmitis, and the most commonly employed intracameral antibiotic in the U.S. is vancomycin. There is now evidence that intracameral vancomycin is most likely associated—although extremely rarely—with hemorrhagic occlusive retinal vasculitis (HORV). This issue starts with David F. Chang, MD, and Andre Witkin, MD, detailing the concerns at hand and the work of the ASCRS/ASRS Task Force on HORV. We also share the complete joint statement of the ASCRS/ASRS Task Force, as well as some opinions on the findings from 4 leading surgeons, Neal Shorstein, MD, Richard Kent Stiverson, MD, Jeffrey Liegner, MD, and Richard Mackool, MD.
