Dr. Devgan is a fan of the "sim- ple iris hook." He currently prefers reusable hooks as they're a "fantastic deal financially for the surgery cen- ter. The huge benefit for surgeons is the variability of placement. You can change the pupil size to whatever you desire—a Malyugin ring has to be at 6 mm," Dr. Devgan said. In cases of weak zonules, Dr. Devgan will use hooks to support the capsule as well as the iris, he said. As for detractors, "iris hooks take a little longer to put in the eye," Dr. Devgan said. "But when it's a complicated case, I don't mind spending the extra time. Another detractor that's been cited is the need for an extra incision. But as I. Howard Fine, MD, taught us, there's no penalty for making a small paracentesis or even multiple paracenteses in the eye. The incision size for iris hooks is 0.5 mm." New devices Among the "interesting" newer devices is the Bhattacharjee ring, Dr. Devgan said. The ring can ex- pand up to 6.5–7 mm, but can also be manipulated into a hexagonal shape to give surgeons a little more working room, he said. Beaver- Visitec (Waltham, Massachusetts) introduced the I-Ring Pupil Expander that's made out of plastic and designed to give surgeons "an enlarged, round pupil," Dr. Devgan said. The Assia Pupil Ex- pander (APX), developed by Ahud Assia, MD, goes through a paracen- tesis to hold the pupil open. "It comes down to what's right for your technique, your surgery center, and most of all what's right for the patient," he said. "Every case is individual, and there's no one single way to treat all cases," Dr. Lewis said.
