The new low-add Tecnis Mul- tifocal IOLs increase our near and intermediate focal length choices, and they improve the night vision symptom profile—all while main- taining the quality of vision at the near focal range. Halos around lights at night are a major contributor to night vision complaints. With multi- focal IOLs, halos are the result of the defocused near image on the retina, so halo size is related to add power. Therefore, lower add powers create smaller halos, which appear to be less distracting and better tolerated than larger halos. Post-approval clinical experience After FDA approval, I thought that I would be using different add powers of the Tecnis Multifocal IOL in con- tralateral eyes (custom matching) to best achieve patients' goals. While I have done this, I have found that this has typically not been necessary. My approach is to make an initial determination of the best add power based on each patient's height, arm length, and lifestyle. I give them a preoperative ques- tionnaire to identify desired vision, occupation, hobbies, and computer use. I also give them something to read and observe where they hold the reading material (a tall person with long arms will hold it farther out than a shorter individual, for example). I then implant the lens— and it is typically the +2.75 ZKB00— in the first eye, and then I make the second eye determination based on feedback on the first. Most patients like the near vision and night vision profile, so I end up putting the same lens in both eyes. Occasionally, patients tell me they wear reading glasses for very small print, but most seem happy with this and otherwise do not need any correction. I am more confident offering the low-add ZLB00 and ZKB00 lenses to younger cataract surgery and even refractive lensectomy patients who are still working and who are reliant on computers and tablet devices. The expanded range of add powers, coupled with a more desirable night vision profile and the same pupil independence and quality of vision we have come to expect from this platform, gives me greater confi- dence in meeting patients' near and intermediate vision goals.
