"We recommend this new tech- nique for patients who have posteri- or capsule rupture in 2 cases: when there is a great hypotonia or when there is a vitreous loss with a big posterior capsule rupture," he said. "The bubble takes away the hypo- tonia and makes … the phacoemul- sification and IOL implantation [easier]." He also highlighted cases where surgeons cannot use the bubble, such as cases of small posterior cap- sule rupture because it will worsen the rupture. It is also not recom- mended when there is no vitreous loss, he said. Eduardo Martinez, MD, Largo, Fla., presented the best case of the Refractive Session IV, about post- LASIK residual hyperopia. It focused on a patient who underwent LASIK and whose cornea became too steep. "We had to flatten the cornea with- out inducing hyperopia," he said. "I removed the lens, leading to myo- pia, and then I flattened the cornea with the excimer [laser], leading to myopia correction." Best cases of the meeting and awards The last day of the meeting was marked by the Organizing Commit- tee awarding the best cases present- ed throughout the congress. The Or- ganizing Committee offered awards to the 8 best scientific cases of the meeting. Bruno Fontes, MD, Rio de Janeiro, Brazil, won the best of all in the refractive surgery session, with a case on intracorneal hemor- rhage, and Roberto Coelho, MD, Sao Paulo, Brazil, won in the cataract surgery session, with a case focusing on what to do in high myopia plus cataract and advanced keratoconus. A session on "Successful Brazilians Living Abroad" was also featured and included talks on Brazilian ophthalmologists with established careers in Europe and the United States. Liliana Werner, MD, PhD, Salt Lake City, Utah, discussed the problem of late displacement of an intraocular lens within the capsular bag, which is increasing in frequen- cy. "This issue has motivated a number of studies carried out in our center," she said. Dr. Rocha presented the main points of her research related to presbyopia and pseudoaccommoda- tion, analysis of dynamic wavefront, anterior segment images, and corne- al biomechanics. Finally, Alessandra Chaves, MD, Louvain-la-Neuve, Belgium, talked about choosing intrastromal rings or crosslinking and discussed the minimum ages for each proce- dure and the possibility of combined procedures. In true Brazilian style, everyone came out at night to dance. Whether it be the 70s' tunes of Disco Evolution or Festa Na Roca country folk party, they all had a great time regardless of age.
