The SGR is repealed and replaced with 5 years of 0.5% updates. The new law maintains a fee-for-service option and consolidates existing quality reporting programs into a new Merit-Based Incentive Payment System (MIPS) starting in 2019. Through the MIPS, physicians will receive a composite score for performance based on benchmarks and will receive a bonus, penalty, or no payment adjustment. Additional bonuses are available for exceptional performance. In addition, the law prevents the Centers for Medicare and Medicaid Services (CMS) from proceeding with its policy to transition all 10- and 90- day global codes to 0-day codes, which would have had a serious negative impact on ophthalmology.
