The evolution of endothelial keratoplasty and use in complicated cases Nicoletta Fynn-Thompson, MD, Boston, gave attendees an update on endothelial keratoplasty (EK). It is indicated for treatment of cor- neal endothelial dysfunction, she said. The benefits of EK compared to penetrating keratoplasty (PK) include a faster recovery, more stable and predictable surface topography, minimal change in corneal topog- raphy, and relatively predictive refractive shift, she said. Advantag- es of EK include a reduced risk of globe rupture, lower rejection rate, far fewer sutures, and less potential complications. Dr. Fynn-Thompson also men- tioned the types of EK that have emerged over the years, including DSEK (Descemet's stripping endothe- lial keratoplasty), ultra thin DSEK, DMAEK (Descemet's membrane automated endothelial keratoplas- ty), DMEK (Descemet's membrane endothelial keratoplasty), and PDEK (pre-Descemet's endothelial kerato- plasty). DSEK is still the most widely performed variation of EK, she said. With EK, there are some eyes that can be challenging to handle, and Dr. Fynn-Thompson stressed that eyes with hemorrhage/bleed- ing or eyes with previous glaucoma surgery may require special atten- tion. For patients with an increased risk of hemorrhage, make sure you review preoperative charts, she said. Minimize the damage to the iris root, and minimize disruption of the conjunctiva. Aim to keep the eye pressurized, she said, because soft eyes will bleed more. Additionally, she said to not disrupt the clot if not necessary. For patients undergoing EK with previous glaucoma surgery, Dr. Fynn-Thompson said that glaucoma has the greatest risk for graft failure. Managing the air bubble can be challenging in these cases. Proper placement of the graft may also be difficult at times, and previous surgery and scar tissue may dictate the location of the wound and tissue delivery system you use, she said. EK continues to evolve, Dr. Fynn-Thompson said. Better tech- niques will provide better outcomes, which will allow for more complex cases with other co-morbidities to be addressed. Adapting to the challeng- es of each case will provide the best possibility for long-term endothelial graft survival, she said. Editors' note: Dr. Fynn-Thompson has no related financial interests. Using intraoperative aberrometry to improve prediction error Stephen Brint, MD, Metairie, La., gave a talk titled "Retrospective Analysis of Prediction Error of IOL Calculation in a Large Population of Post-Myopic LASIK Patients Under- going Cataract Surgery." IOL power calculation has traditionally been challenging in post-refractive surgery eyes, Dr. Brint said. He discussed a large population study that evaluated the impact of intraoperative aberrometry on the prediction error of power calculation for post-myopic LASIK eyes.
