Although the cost of the lens— approximately 1,000 Euro (1,446 U.S. dollars)—is a definite drawback, patients getting the LAL are pre- pared for the expense, Dr. Neuhann said. "When I compare this to fem- tosecond phaco, which is also inter- esting and where the patient pays more, I think the patient investment is similar, but the outcome is more valuable from the LAL than from femto phaco," he said. Special uses Both Drs. Neuhann and Dick are using the 2- to 3-week post-op pe- riod to try monovision in a number of patients. "You can try it for 2 weeks and judge if it's good for them or not. I do the dominant eye for distance and the non-dominant eye for near, with about 1.5 D in be- tween. This is something unin- tended with the LAL but it's a strength [to use it this way]," Dr. Neuhann said. Post-refractive patients can also benefit from the LAL. "We have 20 different formulas for these patients, and there's no formula you can use in all cases for perfect vision," Dr. Neuhann said. "These patients tend to be demanding, and they want everything. It's nearly impossible to promise this, but with the LAL, they can get perfect vision." In his stud- ies, Dr. Neuhann has not had to sep- arate post-refractive patients into a different study group than standard patients as the post-op results are so similar. An international post-refractive cataract study group in which Dr. Dick participates includes 37 eyes, with 20 eyes targeted for em- metropia and 17 eyes targeted for adjustable monovision. The average post-op adjustment was 0.88 D in these patients, with an overall im- provement in all cases of more than two lines. All eyes are able to see bet- ter than 20/40, and 50% are 20/20 or better, Dr. Dick said. Although Calhoun Vision does not recommend using the LAL in pa- tients with AMD or glaucoma, Dr. Neuhann has some anecdotal results from three patients with AMD. For example, in one patient with a ma- ture cataract and AMD that was hard to detect initially, the post-op result was 20/25, he said.
