EW OB 64 January 2011 by Riva Lee Asbell Coding update for Medicare—2011 INTRODUCTION A Happy Healthy New Year to all! There are significant CPT code changes this year, particularly the ones regarding OCT (Scanning com- puterized ophthalmic diagnostic im- aging—SCODI). Category III codes become effective every six months. Highlights of the changes fol- low. In CPT a new code for that year follows the red bullet (—). Com- ments that are printed in green with text inserted between „ƒ are new material for 2011. The p symbol in- dicates the code description has been revised from the previous year. When there is an indent, all of the code description up to the semi- colon is included in the official de- scription as in code 92134. NEW CPT (Current Procedural Terminology) CODES —65778 Placement of amniotic membrane on the ocular surface for wound healing; self-retaining —65779 single layer suture —66174 Transluminal dilation of aqueous outflow canal; without retention of device or sent —66175 with retention of device or stent —92132 Scanning computerized ophthalmic diagnostic im aging, anterior segment, with interpretation and report, unilateral or bilateral —92133 Scanning computerized ophthalmic diagnostic im aging, posterior segment, with interpretation and report, unilateral or bilateral; optic nerve —92134 retina „(Do not report 92133 and 92134 at the same patient en- counter)ƒ„(92135 has been deleted)ƒ „(for scanning computerized oph- thalmic diagnostic imaging of the optic nerve and retina, see 92133, 92134)ƒ The significant change from CPT code 92135 to codes 92132, 92133 and 92134 is that SCODI is now paid once for both sides whereas it was formerly paid for each side. —92227 Remote imaging for detection of retinal disease (eg, retinopathy in a patient with diabetes) with analysis and report under physician supervision, unilateral or bilateral „(Do not report 92227 in conjunc- tion with 92002 – 92014, 92133, 92134, 92250, 92228 or with the evaluation and management [codes]of the single organ system, the eye, 99201 - 99350)ƒ —92228 Remote imaging for monitoring and management of active retinal disease (eg, diabetic retinopathy) with physician review, interpretation and report, unilateral or bilateral „(Do not report 92228 in con- junction with 92002 – 92014, 92133, 92134, 92250, 92227 or with the evaluation and management [codes]of the single organ system, the eye, 99201 - 99350)ƒ DELETED CPT CODES 92135 Scanning computerized ophthalmic diagnostic imaging, posterior segment, (eg, scanning laser), with interpretation and report, unilateral 0016T Destruction of localized lesion of choroid (eg, choroidal neovascularization), transpupillary thermotherapy 0017T Destruction of macular drusen, photocoagulation 0176T Transluminal dilation of aqueous outflow canal, without retention of device or stent 0177T with retention of device or stent 0187T Scanning computerized ophthalmic diagnostic im aging, anterior segment, with interpretation and report REVISED CPT CODES p65780 Ocular surface reconstruction; amniotic membrane transplantation, multiple layers p66761 Iridotomy/Iridectomy by laser surgery (eg, for glaucoma) (per session) CATEGORY III (EMERGING TECHNOLOGY) CODES The Category III codes are actually updated every 6 months and can be accessed online at the AMA website www.ama-assn.org. This is some- what confusing due to the vagaries of the system. The implementation date occurs six months after the re- lease date. The appearance of the code in the CPT book depends on the cycle. You may begin using the code on the implementation date even though it does not appear in that year's CPT. On January 1, 2011 codes will be released that you may begin using July 1, 2011. The following changes appear in CPT 2011. The Sunset with a date is the year the code will no longer be valid unless it is brought forth to the CPT Editorial Panel and becomes ap- proved for a Category I code. p0191T Insertion of anterior segment aqueous drainage device, without extraocular reservoir; internal approach, into the trabecular meshwork Sunset January 2014 — 0253T internal approach, into the suprachoroidal space Sunset January 201 NEW ICD-9 CODES New Codes and Categories New Category: V90 Retained foreign body Embedded fragment (status) Embedded splinter (status) Retained foreign body status Excludes: artificial joint prosthe- sis status (V43.60-V43.69) foreign body accidentally left during a procedure (998.4) foreign body entering through orifice (930.0-939.9) organ or tissue replaced by other means (V43.0-V43.89) organ or tissue replaced by transplant (V42.0-V42.9) personal history of retained for eign body removed (V15.53) superficial foreign body (splin ter) (categories 910-917 and 919 with 4th character 6 or 7, 918.0) New V90.1 Retained metal subcate- fragments gory: Excludes: retained radioactive metal fragments (V90.01-V90.09) New code V90.10 Retained metal fragments, unspecified Retained metal fragment NOS New code V90.11 Retained magnetic metal fragments New code V90.12 Retained nonmagnetic metal fragments New code V90.2 Retained plastic fragments Acrylic fragments New V90.3 subcate- Retained organic gory: fragments New code V90.33 Retained wood fragments New code V90.39 Other retained organic fragments New V90.8 subcate- Other specified retained gory: foreign body New code V90.81 Retained glass fragments New code V90.83 Retained stone or crystalline fragments Retained concrete or cement fragments New code V90.89 Other specified retained foreign body New code V90.9 Retained foreign body, unspecified material
