"In terms of the toric option, we do all the routine tests, which in our case includes the IOLMaster [Carl Zeiss Meditec, Jena, Germany], a determination of axial length and astigmatism; we also measure with corneal topography the autoker- atometry and the current refrac- tion," he said. "All those things together will point the way to the determination of the power of the lens as well as the cylinder of the lens." One key is to get good preopera- tive measurements, which involves using multiple devices and making sure that the measurements prior to surgery are accurate. Determining axis and power is very important, Dr. Lewis said. Currently there are several toric IOL options, including the AcrySof Toric IOL (Alcon, Fort Worth, Texas), STAAR Toric IOL (STAAR Surgical, Monrovia, Calif.), Tecnis Toric IOL (Abbott Medical Optics, Santa Ana, Calif.), and the Trulign Toric poste- rior chamber IOL (Bausch + Lomb, Rochester, N.Y.). "What I would encourage [physicians] to do is get comfort- able," he said. "They have to get comfortable with all of the technol- ogy behind this." Dr. Lewis said it is important to be able to offer every patient, including the glaucoma patient, the best potential for visual improvement.
