A S C R S World view EyeWorld in 2014 W e begin the New Year with a special cover focus on phaco and vitrectomy, and we have asked both retina and cataract experts to opine on a number of practical clinical questions. Mike Lahey, MD, Steve Safran, MD, and Kevin Miller, MD, debate the timing of planned vitrectomy and phaco— should the surgeries be combined or sequenDavid F. Chang, MD, tially staged? The controversial decision of EyeWorld chief medical editor whether to perform anterior vitrectomy via the limbus or the pars plana is discussed by Garry Condon, MD, Steve Dewey, MD, and the "other" Howard Fine, MD, a Rutgers vitreoretinal specialist. Special considerations for performing phaco in post-vitrectomized eyes are addressed by Nicole Fram, MD, Rosa Braga-Mele, MD, and Suber Huang, MD, immediate past ASRS president. Finally, Lisa Arbisser, MD, and Mike Snyder, MD, offer their pearls on when and how to use triamcinolone to stain the vitreous and facilitate anterior vitrectomy. This is my opportunity to announce several new EyeWorld columns that will debut in 2014. The Young Physician and Resident (YPR) Clinical Committee, under chair Elizabeth Yeu, MD, will direct a column called "Cataract and refractive surgery fundamentals." In the YPR Committee's survey of recent residency graduates, many respondents professed insufficient training with keratorefractive surgery, refractive IOLs, and astigmatism management at the time of cataract surgery. This column will be devoted to fundamental skills of importance to less experienced surgeons. Another new column will be "EyeWorld academic grand rounds." Each installment will feature a clinical case presentation from a prominent academic ophthalmology program, accompanied by faculty discussion in a grand rounds format. This nicely complements our popular "EyeWorld journal club," in which rotating residency programs review concurrently published JCRS papers of particular clinical importance. EyeWorld will be an integral part of a multi-pronged initiative to target ASCRS education toward topics gleaned from our ASCRS Clinical Survey. More than 1,000 ASCRS members completed the 2013 survey, in which the 89 questions were developed and analyzed by Clinical Committee members. The survey measures opinions and practice patterns relating to the most significant and controversial clinical issues. Ongoing surveys will assist our Clinical Committees to ensure that ASCRS education is "on target" and most relevant to our members. Later this year, look for a regular EyeWorld column devoted to clinical learning objectives that the Refractive and Cataract Clinical Committees have prioritized based on the ASCRS Clinical Survey. Finally, I want to thank the physician editors of our numerous regular columns that offer EyeWorld readers unique educational value year-round. Thanks to Keith Warren, MD, and the Retina Clinical Committee ("Retina consultation corner"), Rich Tipperman, MD ("The art of advanced technology IOLs"), Nick Mamalis, MD, and Liliana Werner, MD ("Cataract/IOL complications: Moran CPC reports"), Steve Safran, MD ("Anterior segment grand rounds"), Rich Hoffman, MD ("Tools and techniques"), Kevin Miller, MD ("Complicated cataract cases"), and J.C. Noreika, MD ("Insights"). Thanks also to Sherleen Chen, MD, and Roberto Pineda, MD ("Cataract tips from the teachers"), Tom Oetting, MD ("Cataract M&M rounds"), and the 30 different residency program directors who have participated in the "EyeWorld journal club." These last three features that are of special interest to residents are permanently archived in the free online ASCRS Phaco Fundamentals Classroom (phaco.ascrs.org). Section editors Reay Brown, MD, Clara Chan, MD, Bonnie Henderson, MD, Kerry Solomon, MD, and John Vukich, MD, join me in wishing you a happy and enlightening 2014. David F.
