Audrey Talley Rostov, MD, of Northwestern Eye Surgeons, is a believer that each and every patient, regardless of means, deserves their best possible visual outcome following cataract surgery. That's why Dr. Rostov uses the LENSAR Laser System with Streamline IV for every one of her cataract patients. She finds that this highly evolved technology provides the precision, accuracy, and confidence she needs to deliver optimum outcomes, particularly for advanced IOL procedures and certain complex cases. In a recent interview, Dr. Rostov delved into the specific features of the LENSAR Laser with Streamline IV that have changed the entire landscape of cataract surgery and driven her to use the laser in every single one of her cataract procedures. Q: Why do you recommend using the LENSAR Laser in all ATIOL procedures? A: Whatever a surgeon's IOL of choice is, LENSAR is the tool that can help you deliver your patients' best possible outcomes. That's because with LENSAR, you have the ability to create a custom capsulotomy so you can nail it and get the effective lens position where you want it, each and every time. You don't have to worry about the capsulotomy being off of the visual axis. And with IntelliAxis Refractive Capsulorhexis, you don't have to worry about your toric IOL alignment being off. Further, LENSAR's Augmented Reality imaging allows you to visualize the cataract in the capsule and in the eye, so you can know exactly what to expect before beginning the procedure. Together with the precision capsulotomy capabilities, this helps you deliver the optimum, best possible outcomes for your patients, no matter what advanced IOL you're implanting. Q: How does LENSAR help you achieve optimum outcomes in your most complex cases? A: LENSAR changes the entire landscape of cataract surgery because it gives me the confidence to transform even the most complex case into one that is routine. This is possible because of how LENSAR can create a capsulotomy, even in a dense, black cataract or a completely white cataract, knowing it will have perfect centration and optimal effective lens positioning. "LENSAR changes the entire landscape of cataract surgery because it gives me the confidence to transform even the most complex case into one that is routine." Audrey Talley Rostov, MD Sometimes it is the patients with these complex cases and those who are suffering from cataract-related blindness who can afford femtosecond cataract surgery the least but who need it the most. These patients, and all of my other patients for that matter, deserve their best possible outcomes regardless of circumstance. This is why I have elected to use LENSAR in each and every case. Q: What is it about the LENSAR Laser that makes it particularly helpful for toric IOLs? A: LENSAR is an amazing tool for cataract surgery, particularly for toric IOLs and toric IOL alignment. It gives you the ability to seamlessly integrate your diagnostic data from your favorite device directly into the laser. This eliminates manual entry and any error that can originate in this step, which gives you more confidence that the axis you want to align with will be there. Second, IntelliAxis Refractive Capsulorhexis allows you to make a perfect capsulotomy every time with tabs that accurately mark the axis for toric IOL alignment. This unique feature eliminates errors that can result from manual ink marking and cyclorotation. In the OR, these marks can be used with confidence knowing they are placed accurately. And then at the slit lamp, the marks can be used to tell whether or not the IOL is on axis. The net effect is that toric IOL alignment becomes easy and straightforward so you can deliver the best possible outcomes. Augmented Reality creates a 3-D model of the eye for visualization of the patient's anterior anatomy prior to the laser procedure. IntelliAxis Refractive Capsulorhexis (shown in blue) includes marks that indicate the steep axis of astigmatism, which can be used for alignment in the OR and at the slit lamp post-op. Q: What would you say to surgeons who don't believe a femtosecond laser is necessary in every cataract case? A: Every patient deserves their best possible visual outcome. In my experience, the only way to achieve the best outcome for every single patient is to use a femtosecond laser, specifically the LENSAR Laser System with Streamline IV. Creating a perfect capsulotomy and subsequently achieving effective lens positioning is critical for achieving this type of outcome. With LENSAR, you know the capsulotomy is going to be centered, giving you the effective lens positioning you need. It's hard to compare this type of precision to what's achievable in a manual procedure.
