In his second talk, Prof. Seiler cautioned against doing PRK and CXL at the same time. Although the combined procedure saves time and limits the patient���s pain to only once, it has poor predictability in 40-65% of cases. Crosslinking and photoablation after the cornea is stable offers the best predictability, Prof. Seiler said. The refractive effect of crosslinking isn���t predictable and neither is regression, which could continue for years, Prof. Seiler said. He also cautioned against the epithelium-on approach. ���Until we get the right formulation for transepithelial CXL, we will stay with the epithelium-off because then we know what we���ll get,��� he said. Prof. Seiler reported on alternatives to crosslinking. For instance, researchers saw a stiffening effect in porcine and rabbit eyes by using glutarhide and Karnovsky solutions. Sunlight seemed to have the highest effect on stiffening. Type 1 diabetes, too, was shown to be a protective factor for keratoconus. Twenty days of glucose and ribose in the eyes creates a crosslinking effect, Prof. Seiler reported, but ���leaving glucose in for 20 days is very hard to do.
