Doing more toric lenses we always wonder, will we have some residual left over, did we accidentally miss the axis to counteract it, or did the lens rotate? What I love is that the OPD-Scan III has an internal OPD map. It quickly shows me where the lenticular astigmatism is–even on a small pupil, without dilating the patient ...making my life a lot easier. Toby Tyson, MD Tyson Eye, FL If patients have irregular astigmatism then I'm always thinking about corneal dystrophy, epithelial basement membrane dystrophy, because typically they'll have some subtle EBMD, OSD, or dry eye. It's nice to be able to show a patient their OPD scans, so they can see the surface irregularity that needs to be addressed. If there's a bowtie, I say, "do you see the bowtie…that's your astigmatism…so, a toric lens may be the ideal option for you because this bowtie is really big". If the astigmatism pattern is non-symmetric, the patient may not be a good toric IOL candidate. Whether it's corneal scraping or another technique, it is essential to be able to show patients the before and after images to see how irregular the corneal surface is, why we need to remove this bad tissue, let it heal, delay 3-months and get it right. Even a half diopter of astigmatism could lead to unhappy visual results. The OPD really helps delineate the source of sub-optimal vision, in the pre- and post- operative period.
