Similarly, a series of consensus discussions were held to determine a set of risk factors. The study recommended the preoperative recording of demo- graphics, ocular history and comor- bidities, preoperative visual acuity, and patient reported visual function. The recommended outcomes were split into intra- and postoperative metrics. Intraoperative outcomes included capsule related problems, dislocation of the lens nucleus fragments into the vitreous, and other complications. Postoperative outcomes included visual acuity, re- fractive error, patient reported visual function, and early and late compli- cations of surgery. "ICHOM, like PROMIS, empha- sizes patient reported outcomes," Dr. Alonso said. "Patient outcomes are what we value most. To qualify the care of conditions, the overall best strategy is to use basic clinical outcomes and the patient reported outcomes, and divide by the cost to make decisions. PROMs give us informative, precise data, and we must start to use them. We need to aim for common/interchangeable PROMs that can be used in different populations or types of patients. ICHOM Patient reported outcomes are just one way to assess treatment meth- ods. Another approach focuses on outcomes as the ultimate measure of success in healthcare. ICHOM was designed to transform healthcare systems worldwide by measuring and reporting patient outcomes, per medical condition, in a standardized way. This allows patients to better predict and understand the effect their treatment will have on their lives and livelihood. "ICHOM was promoted by several institutions and academic advisors with the idea to unlock the potential of value based healthcare by defining global standard sets of outcome measures for the most relevant medical conditions," Dr. Alonso said. "This information really matters to patients." ICHOM's current activities include defining standards and setting benchmarks on outcomes from practice, not from clinical trials or efficacy studies. "ICHOM measures outcomes, collaborates to improve value, and develops value based payment models. It facilitates adoption and implementation by sharing knowledge and supporting proof of concept. By engaging payers and governments to realign finan- cial incentives, it hopes to promote transparency," he said. Twenty-one ICHOM standard sets have been provided to date, including standards sets for cat- aract and macular degeneration that include major indicators and surgical complications, visual acuity, refractive error, and patient reported visual function. Aligning outcomes worldwide could facilitate inter- national comparisons and allow improvements in the outcomes most relevant to patients.
