Pa- tients are incredibly grateful at being less dependent on their glasses or contact lenses. There is a caveat, however, to the area of refractive sur- gery—the notion that anatomical success has very little meaning to our patients. Telling pa- tients, "The lens is in an excellent position" or "Your LASIK flap looks pristine" has little mean- ing when patients spend much of their day squinting and struggling to see well enough without their glasses. This is true for all refractive surgery patients, routine and complex. There is an- other caveat: Not every well thought-out surgical plan works out exactly as it should, even when everything is done according to plan. This month's issue focuses on the management of challenging refrac- tive surgery cases. In one feature article, a toric IOL case is presented where everything went according to plan, yet challenges and post-op surprises still occurred. This is the nature of refractive surgery. Members of the refrac- tive editorial board Neel Desai, M.D., and John Berdahl, M.D., offer tips and pearls to readers facing similar cases in their own practices. In another feature, Dr. Berdahl, John Kanellopoulos, M.D., and Richard Lindstrom, M.D., discuss the challenges facing surgeons when performing refractive surgery on post-keratoplasty patients. Additionally, Parag Majmudar, M.D., Eric Donnenfeld, M.D., and John Vukich, M.D., offer pearls and tips for managing post-radial keratotomy (RK) hyperopic shift. As we are aware, post-RK patients can present common challenges for all corneal and cataract refractive surgeons. Finally, Karl Stonecipher, M.D., and Dr. Lindstrom provide their counsel and suggestions for managing the unhappy patient who returns months to years after surgery.
