It allowed for distance, near, and intermediate vision for these young patients clearly in need of visual assistance for their previously debilitating lens conditions. Further, while the incidence of Alport syndrome is low, you might encounter it more often than you think. "We want to emphasize that you might be missing these patients when they come to your clinics," Dr. Goel said. "The incidence could be underreported." Dr. Goel's argument could be buttressed by the fact that only anterior lenticonus appeared in these patients as opposed to the systemic classic signs. Only a dilated slit lamp examination—not always performed prior to refractive surgery—would almost guarantee the observance of lenticonus, he said. "The typical droplet sign can be made out in a dilated exam upon retroillumination," Dr. Goel said. Multifocals for single cataract patients Apart from Alport syndrome—which is rare—multifocal IOLs come in handy for another debilitating visual condition—unilateral cataract in the young population. "All young patients with cataracts in one eye are patients who need multifocal IOLs," Dr. Goel said. Whether these unilateral cataracts are due to trauma, a disease, medication, or other reasons, patients need multifocal IOLs in such instances to work together with their normal eye. "Two eyes go together both for distance and near vision," Dr. Goel said. "Both eyes mix very well and don't have any issues." In using the term "mix" Dr. Goel harkened back to the debate over whether ophthalmologists could "mix and match" two different multifocal or accommodative IOLs, with a different type used in each eye—a controversial practice. "But there are no issues in patients with a multifocal in on eye and a natural lens in the other eye," Dr. Goel said. "The normal eye works for all distances." Lesson learned: While multifocal IOLs still may be touted as "premium," they are nothing short of EW International critical to restore sight in the most prudent manner for a great number of patients in India and beyond.
